What specifically causes cold sores?

Cold sores are a symptom of the herpes simplex virus. There are actually two kinds of herpes simplex virus, "type one" and "type two".

Type 1 herpes simplex virus.
Herpes simplex virus type 1 (HSV1) generally only infects those body tissues that lie "above the waistline" and it is HSV1 that causes cold sores in the majority of cases.

Type 2 herpes simplex virus.
Herpes simplex virus type 2 (HSV2) commonly infects the genital area (genital herpes). Although Herpes simplex virus type two can cause cold sores, it is a less common occurrence than with HSV1.
